An Access Page can be faxed directly to a care provider by our facility or provided to a patient at the time of their visit to be delivered to their physician/provider. Once an Access Page has been received, the care provider can easily view or import the shared imaging in full diagnostic quality.
For personal access to your imaging records, you can enroll online by visiting THIS LINK or clicking the button below. Once you register for access, you will receive your available imaging history from Munising Memorial Hospital and other affiliated facilities within your PocketHealth account! FEES
You can also share imaging instantly with any physician or even burn your own CD/USB from home. There is a fee at the time of enrollment which includes permanent access to all previous imaging as well as any new exams. PocketHealth has two membership options: Flex ($10/month, cancel anytime) and Unlimited ($49/year). Both plans give you:
- Unlimited imaging records requests from any of the 600+ PocketHealth-connected hospitals and clinics
- Up to 4 additional profiles to manage all imaging records, all in one place
- Lifetime permanent storage for all imaging records
- Ability to cancel anytime and continue to access and share your imaging records, from any device, without incurring any additional charges
Exclusive to the Unlimited Plan is the ability to securely store and share any past imaging from CDs and any health records, including lab results, prescriptions, and vaccine receipts.

Patients may exercise the right to opt out of access to medical records via PocketHealth at any time. By consenting to access records via PocketHealth, patients waive all claims against Munising Memorial Hospital, its physicians, employees, affiliates, and agents for all purposes in connection with said communication and disclosure of information in the said records.
Munising Memorial Hospital advises all patients to consult with a medical professional when attempting to interpret or understand their medical imaging history.
