Know Your Rights


As a patient at MMH and/or Bay Care Medical Center you have both rights and responsibilities. The following ‘Patient Bill of Rights’ and ‘Patient Responsibilities’ have been developed for you.

Your Patient Rights

  • You are entitled to considerate and respectful care.
  • You have the right to obtain from your physician, complete and current information concerning you diagnosis, treatment and prognosis in terms you can understand.
  • You are entitled to receive from your physician, information necessary to give informed consent before the start of any procedure and/or treatment.
  • You are entitled to know how a complaint can be made and how it will be resolved by the hospital and you need not fear about making a complaint or comment.
  • You have the right to privacy concerning your own medical care program.
  • You have the right to expect all communications and records pertaining to your care will be treated as confidential.
  • You have the right to receive and examine an explanation of your bill, regardless of the source of payment, and to receive, upon request, information about financial assistance through the hospital.
  • You have the right to refuse treatment to the extent provided by law and to be informed of the consequences of the refusal. When refusal of treatment prevents the hospital or its staff from providing appropriate care according to ethical and professional standards, the relationship with you may be ended upon reasonable notice.
  • You are entitled to information about hospital rules and regulations affecting your care and comfort.
  • You have the right to medical care free of discrimination on the basis of race, creed, religion, sexual preference, color, national origin, sex, age, economic status or handicap.
  • You have the right to prepare an “advance directive” for medical treatment and to appoint a surrogate to make treatment decisions for you to the extent permitted by law. The presence of an advance directive is not a requirement for care at MMH.
  • You have the right to access, within the limits of the law, information contained in your medical record.
  • You are entitled to be informed of continuing health care requirements following discharge from the hospital including time and location of follow-up appointments and the identity of the person providing care.
