The diabetes self management program is designed to help individuals manage their disease. It includes a diabetes assessment, with a Certified Diabetic Educator, to develop an individualized self-management plan. The Medical Nutritional Therapy program provides up to 3 hours of nutritional counseling for persons with diabetes or kidney disease.
Our program is certified by the American Diabetes Association and the State of Michigan. This program offers both group and individual appointments. Registered Dietitian/Coordinator: Paula Ackerman, MS, RD, CDE. Individual assessments are held in the Outpatient Rehab department at MMH.

To register for diabetic education services, please go to the Admitting Office located by the front lobby entrance. You do not need to register at the Admitting Office for any other appointments.
Insurance companies are recognizing the importance of the Diabetes Education to help prevent long-term complications. Most insurance companies cover a portion or most of the cost after your deductible is met. Check with your insurance company to see if they cover Diabetes Education. If you do not have insurance, please call to find alternative ways to receive Diabetes Education. We will be glad to assist you.

MMH Diabetes Support Group meets monthly in the MMH Maple Conference room. We discuss various topics in diabetes care and treatment. The support group is free of charge and open to the public.
